The challenge
Meridian’s legacy patient portal was a PHP monolith that took 12 seconds to load and crashed during flu-season spikes. Patients booked appointments by phone because the web experience was unusable. The system needed to be HIPAA-compliant, accessible, and able to handle 10x traffic bursts.
The solution
We built a Next.js portal with edge rendering, a type-safe tRPC API, and a React Native app with offline-first sync. Appointments, messaging, and records moved behind a role-based access control layer with full audit logging. We introduced a design system and visual regression tests to keep the experience consistent across web and mobile.

Patients now book in seconds, not minutes. Our call center volume dropped by half in the first quarter after launch.
